Tomball Regional Medical Center Home Health is a hospital-based home health agency that provides patients with high-quality, personalized care while helping them maintain their independence at home.

Our Home Health services comprise a wide range of health and social services, including:

  • Skilled Nursing, such as cardiac, respiratory and diabetic care and management, and wound care.
  • Physical Therapy, such as building strength and mobility and gait training.
  • Occupational Therapy, such as increasing independence in daily activities and retraining motor skills.
  • Speech Therapy, such as communication improvements and swallowing therapies.
  • Medical Social Services, such as referrals for community resources, financial aid and counseling services.
  • A Home Health Aide to assist with bathing and grooming (Available only when receiving one of the above services).

Our staff of experienced caregivers includes professional nurses, physical therapists, occupational therapists, speech therapists, social workers and home care aides. A registered nurse or physical therapist visits patients in their homes and develops a plan of care with the patients’ physicians.

Falls Hurt!

falls hurtThe TRH Home Health Education on Fall Prevention objective is to inform and educate the patients and family about the causes of falls in older adults, and the assessment, rehabilitative, and health promotion measures which can be taken to reduce the risk of falls in this age group.

TRH Home Health is accredited and regulated by federal and state agencies. All of our home health staff undergo thorough screening, including background checks. Medicare, Medicaid and commercial insurance plans usually cover TRH Home Health services. We provide services without regard to race, color, sex, national origin, age or handicap.

Although a physician order is required to initiate home care services, referrals and additional information can be obtained through your hospital discharge planner, your physician's office staff, a medical social worker or TRH Home Health.
